Aviation Medicine Consultation

Encounters that do not require a physical exam or diagnostic testing; that will not be added to the airman's FAA medical record.
Comprehensive Sports Assessment

Comprehensive pre-participation sports assessment including exam, baseline concussion testing, ECG, and wellness-risk assessment
Sports Acute Illness/Injury

Problem-focused assessment, treatment and/or referral for sports-related illnesses & injuries (including concussion) using evidence-based return-to-participation checklists & protocols
Sports-Related Consultation
$30
Encounters that do not require a physical exam or diagnostic testing; including sports/camp health forms & NCAA/NAIA Therapeutic Use Exemption (TUE) documentation.
Non-Sports Illness/Injury
$50
Problem-focused evaluation, treatment and/or referral for non-sports-related illness & injuries occurring at school, work and/or leisure. Pricing includes all point-of-care diagnostic testing that can be performed in the office
